
The BC Sentinel clinical application allows personalized monitoring of users who receive care from the different devices of the Home Care service, such as home hospitalization or PADES. The project has received several awards in the field of innovation, including the e-Health Grant from the Col·legi de Metges de Girona or the 3rd CSC Impulsa Award 2022 from the Consorci Social i de Salut de Catalunya The transversality of the project is sought by working to export it to other teams and services of the Corporation such as Primary Care or Pharmacy April 26, 2023 — The Comprehensive Home Care (ADI) service of the Corporació de Salut del Maresme i la Selva, a public health entity that manages several health centers, including the hospitals of Calella (Barcelona) and Blanes (Gerona), together with Better Care, a biotechnology company that develops software solutions based on artificial intelligence for continuous patient monitoring, have created a digital platform, BC Sentinel, a clinical application for monitoring and home patient management that allows the automatic and real-time integration of the data recorded by medical devices and the recording of drug administration. The platform brings together data from various sources: those from the medical record, those entered by ADI professionals and those generated by the users themselves from their homes using an application. Thus, health professionals have a large amount of information to develop a clinical practice of the highest quality. This initiative, which has already received several awards, wants to be exported to other teams and services of the Corporation, such as Primary Care, Pharmacy or Outpatient Consultations.
BC Sentinel consists of an mHealth web platform that professionals can consult from the office using the computer or from their homes through tablets with an internet connection. In addition, it has a mobile application that users can download for free and that health professionals activate and configure according to the admission profile. In this APP, users periodically enter their vital signs and respond to the clinical questionnaires that professionals have assigned based on their profile and that provide them with information such as the monitoring of PREMS and PROMS or an automatic calculation of scores and graphs of the evolution of the patient's condition. All the data generated from the different devices (thermometer, blood pressure monitor, saturator and balance) are entered through the Bluetooth® system into the APP and are directly integrated into the user's medical record.
Thus, the information is available in real time to all healthcare professionals both within the Corporation and outside the organization who have to care for these people. In this way, users take an active role in their health process and are not just recipients of services. In addition, the fact that users use their own mobile devices and do not have to be provided with tablets or other devices makes it much easier to implement the project. Since the project began in November 2022, a total of 182 users have been introduced to the platform, of which 74 have been monitored. Currently, a study is being carried out to evaluate what clinical impact the follow-up that has been carried out through the platform has had on those patients with higher rates of decompensation and readmission due to chronic respiratory disease or mixed decompensations of respiratory and cardiac pathology, once they have received medical discharge.
Currently, all the Corporation's Home Hospitalization professionals use the tool and since it was implemented, a total of 10,310 measurements such as temperature or blood pressure, among others, have already been introduced and integrated into the medical record. BC Sentinel multidisciplinary project has been a co-designed project between Home Care and Better Care. To improve their usability for both professionals and end users, they participated by giving their vision and perspective regarding the user experience, which has made it possible to provide a real solution to the needs detected from the service. In addition, having the participation of end users has made it possible to achieve an APP that is very simple to use and very intuitive, allowing us to bridge the digital divide between users.
It should be noted that more than 80% of patients admitted to home hospitalization in 2022 were over 65 years old, and 30% were older than 85. He has been awarded the 2022 COMG e-Health Grant from the Official College of Metges de Girona (December 22), has been recognized in the 21st edition of the Social and Health Care Research Award of the Mutuam Conviure Foundation (November 2022), has been awarded the 3rd CSC Impulsa Award 2022 from the Consorci de Salut i Social de Catalunya (September 22), he has also received the Grant for Research in Hospital and Social Health Care from the Academy of Medical Sciences and Health of Catalonia i Balearic Islands (April 22) and has received the award for the best research project of the 1st Research Conference of the Maresme i la Selva Health Corporation (October 21).
